I'm currently torn as to which fellowship to apply to after IM residency. Currently debating between Rheumatology and GI. Yes, I realize they are different, but I went into IM loving it all. Even though they are very different from each other, very interested in both.

I was thinking about taking a year off to solidify my commitment to whichever field I choose, make some money, do research, and travel (a tad bit burned out after almost 7 months of straight inpatient blocks. It may possibly be affecting my decision making.)

What are people's thoughts on SNF (skilled nursing facility) work? I heard you can work a schedule similar to a hospitalist's schedule with 7 on/7 off and do well. That might allow for more time to do research and travel. I realize it can potentially hurt my chances at GI...but also potentially bolster it if I get a couple of papers out of it?

Any thoughts/opinions/alternative suggestions are appreciated. Just trying to slice this situation in different ways and see if others come at it with a different perspective.
