SOAP Note Help

This third year of pharmacy school is full of SOAP notes and researching guidelines. Does anyone have good resources as in example SOAP notes or good places to look up the most current guidelines? Would greatly appreciate it and I'm sure there are others out there in need of help.

Sanford guide to antimicrobial therapy, CDC, and micromedex is what I have used mostly so far for soap notes. I havent heard anyone mention a single place where you can look up different guidelines.
