Sodium Escape in Hyperaldosteronism

Hi guys,

I can't figure out why you get sodium escape with primary hyperaldosteronism but not secondary hyperaldosteronism.

Any thoughts?

WHAT do you mean with sodium scape?

primary= the gland got crazy = excess aldosterone

secondary= something is causing high levels of aldosterone= excess aldosterone.

also the renin is different
 
To elaborate, primary high aldo will have high BP independent of renin-angiotensin, hence low renin and hence less PT reabsorption of Na+.

Everything causing secondary high aldo converges on upregulation of renin-angiotensin, which upregulates PT reabsorption of Na+, tending to cancel out the Na+ wasting due to excess MR activation.
