Ok correct me if I'm wrong surgeons in private practice make a deal with a hospital that all the surgical cases at that hospital will be given to them. Plus surgeons get referral from other doctors. How does a new surgeon get his foot in the door? Every town almost already has a private practice surgery business or the hospital has surgeons. So if there is someone already established in private practice how will you get any cases. Also let's say you have a group of small towns they send all patients that need surgery to a larger city. Could you start a practice in one of cities and contract with all the other small town hospitals to send you any surgical cases. Lastly do you have more power in a private practice. Such as not having vacation limits. Being able to control of your hours more. Etc. I know you'll be working most of the time anyway but it would still be nice to be able to take off when you want to sometimes. PLEASE PLEASE HELP NOBODY WILL ANSWER MY OTHER THREADS AT LEAST TRY AND ANSWER IM CRAVING KNOWLEDGE!!!

You are wrong with how practitioners get cases. Thus, your other assumptions are wrong.

No hospital is going to contract to send all patients to just one surgeon. There is usually a call schedule for the er, but the er or internist can call whomever they want to consult, so the best way to build up your practice is to get the internists to call you for consults.

Whether to participate in insurance programs are a cost benefit analysis for yourself, but being an in network can help as well.

1. Get privileges at hospital(s) you wish to see patients at.
2. Advertise.
3. Meet potential referring physicians to get them to send patients your way.
4. Be willing to take ER/hospital call to get patients.
5. Don't be a jerk to referring physicians or patients.
6. Don't be a jerk to hospital staff and nurses. Their favorable opinion of you goes a long way with potential patients and referring physicians.
 
How you find coverage for weekends and vacations will depend on whether you have partners. Rarely independent surgeons in the community will work out a plan to cover for each other. If you don't have partners then you will need to make sure you have no inpatients when you plan to be unavailable and schedule clinic and cases appropriately. One of my partners closed his practice and joined our group for the sole reason that it was nearly impossible for him to take time off.
