Take a look at a letter recently posted by the ACGME to this place:

Violation of Introduction III. B. 1. : Cases: The case load here is dismal, and residents are often doing cases that are not appropriate for their level of training. CA1s may be assigned VATs, craniotomies, and AAA repairs while CA3s are assigned MAC cases.

Violation of Introduction B.2.b (3): Subspecialty rotations: There is virtually no subspecialty experience. While on paper, different residents are assigned different rotations (peds, CV, neuro), the residents assigned to these rotations rarely do cases for that rotation. The dept. has made no effort to increase the case load for these rotations. Residents posted single digit case loads for their CV rotation, and there is no sign the experience will be different this year. At the same time, no effort is made to send residents to affiliate hospitals (i.e. Westchester medical center). The only rotation residents do outside of St. Vincent’s Hospital is pediatrics for only 1 month during CA3. Residents are given a peds. rotation during CA1 and CA2 years at St. Vincent’s Hospital, even though it is widely known there is insufficient case load at this site for such a rotation. Also, on paper as many as 3 residents will be assigned a CV rotation, even though it is understood that only 7-8 CV cases are going to take place that month in the entire hospital.

Violation of II.A. 4, IV.A.3, and IV.A.5b: Didactics: Didactics exist but mainly in name only. They are frequently cancelled and not rescheduled.

Violation of Introduction II. B. 1. 5A, IV. B. 4: Research: Research has been completely de-emphasized since the last site visit. No attendings except for 1 or 2 even try to do research, and no resident has to do a senior project.

Violation of 5A1b: Resident Evaluations: There is no real transparency in the evaluation process. More than 1/3 of the dept. does not give any feedback, written or otherwise.

Other issues that should be considered include how even though the ACGME guidelines allow for up to 6 months of electives, no resident is allowed to do any elective that would require them to leave the St. Vincent’s ORs, though the affiliations exist

Where exactly on the ACGME website did you find this? That site is so dense to navigate.

The report does hold up to what has been said by posters about the program and also the questions that made residents nervous to answer during the interview.

Should I feel guilty as a CA-1 having done AAA repairs, carotids, thoracotomies, and likely craniotomies next month? Even sat in on a circ arrest case a couple weeks ago. Granted, my CA-3 colleagues are hardly doing MAC cases, but I though early exposure to bigger cases was a good thing?

p.s., I am NOT at St.V

I am very suspect of this reported posting regarding the program at St. Vincent's. The wording of this letter is not consistent with how an ACGME site review report is presented, posted, or worded. The ACGME would never suggest where someone should go and get extra cardiac experience, they would merely cite that the program failed to provide appropriate experience in this or that area. I would look at this very, very carefully and suggest the exact site where this notification finds itself be disclosed and reviewed.

It can be, but some people dont advance at the same pace as others. CA1s should have a strong grasp on the basics of providing a safe anesthetic before moving on to more complex cases. Otherwise you may neglect to learn some of the basics and have to relearn them later. Not neccesarily saying that this is the case for you, but I suspect that it happens.

according to the ACGME website,

Accreditation Status: Continued Accreditation
Accreditation Effective Date: October 25, 2007
Accredited Program Length: 4 years

Program Format: Standard

Last Site Visit Date: July 19, 2007
Cycle Length: 4 years
Approximate Date of Next Site Visit: October 1, 2011

i would not expect a 4 year cycle for a program with the citations listed by the OP.

we're talking about "New York Medical College at St Vincent's Hospital and Medical Center of New York Program" right?

the ACGME website defines continued accreditation as a program "demonstrated substantial compliance with the requirements"

take a look at sp1129's other posts. Clearly there is a anti St V's vibe. Program citations aren't written the way they are outlined here. I call BS.

I wish St.V's posted a list of their residents on the website, that way we could ID this fella with the initials S.P. born on November 29 😀

Also, yes, that wording does not exactly strike one as ACGME-caliber professionalism.

One program I interviewed at in NY actually shows you the letter from ACGME; the letter was nowhere near as long as the OP's comments were, and most certainly didn't have any the "violation of IA.2b.3cXYZ" bullsh*t that the OP posted.

On the other hand, this post wins for the most lame attempt to dissuade folks from ranking a program. Congrats!

SP doesn't have to be a disgruntled resident, maybe this guy/girl is trying to discourage people from ranking this place high so that he/she has a better shot at matching?

In other words, exactly what dan said.

I interviewed at St. Vinnie's last year and I seem to remember similar posts from the OP back then. I could do a search but that would involve work.

St. Vinnie's wasn't too bad from an interview standpoint. Last year they were offering a cool incentive for residents, something like $3-5 for every IV started in the holding area. That's about all I remember since I decided early on Manhattan wasn't for me.

whether or not u believe that the ACGME actually wrote this, you should atleast look at the content of what is being said if you are seriously considering St. Vincents. Are any of these things true? I can tell you that it is right on the money. If you dont beleive me, or sp1129, you should ask anyone at vinnies that u know, or perhaps ask the PD to tell you it isn't true. Do they have cardiac cases like other programs? Have you guys seen a copy of their daily schedule? Without knowing these things, and going into a program without much info is not a wise decision.

that's some pretty amateurish language. Certainly not the ACGME. Can't say the info isn't correct though.
