starting in the urology room...what should I know

Hey all,

I will be starting my CA-1 year on urology cases and am looking for any advice about these types of patients. I have had a long cold intern year with only 5 days of anesthesia so any pearls about what type of cases I can be expecting in the urology suite and things to look out for, things to make sure I do or dont do would be greatly appreciated. Thanks!
 
95% of what you need to know about any surgery, not just urology is: the hole opens, then the hole remains the same and finally the hole closes. Recently had a greenling ortho dude start yappin' about shoulder anchors and my eyes started glassin' over. Many moons ago I pretended to be interested when I could really give a Jody phuck ---now I don't even pretend. Now, 95% of my questions to the surgeons are: Yo slick, how long will it take skin to skin? Regards, -----Zippy

all the holes above the navel are yours, all the holes below the navel are theirs.


some things you're likely to get pimped about - hollow viscous distension, glycine toxicity, water toxicity, lasers, etc.

and if you hear "radical prostate resection using Da Vinci robot..." make sure you take a book into the room to read while you wait.
 
Keep the patient awake, when possible. This is your best monitor for water toxicity/TURP syndrome.

Four steps to standard urology case:

1. Versed
2. Spinal
3. Hyponatremia, TURP syndrome, blah, blah, blah
4. www.break.com
 
Just know TURP syndrome down pat. Also read about Glycine Toxicity from their (surgery) irrigation fluid.

Those are the major topics.
 
we've done away with glycine irrigation fluid.
are people still using it across the board?
