State licensing issue for an IMG

I applied for my state license in April. There is a "Education Verification Form" which I sent to my medical school in India for verification.

However, they can't figure out what to write in the following columns:
1. Total number of weeks of academic instruction completed
2. Total number of weeks of clinical instruction completed

Please help as it's already late and I might not get the trainee license before I start.

Academic instruction = Preclinical, lectures, exams, etc.
Clinical instruction = Time on ward, in the hospital, in the office, etc.
