I was looking at the usmle website, and they mention that after may they make changes to the testing item pool, and therefore delay in results. How significance is this change in item pool? Is it generally associated with lower scores? Or is that a bunch of hocus pocus?

It's almost a yearly thing. There's no association with lower scores, unless you count those that score low and attribute it to the changes.
