hmmm, i thought he remembered mitochondrial...i remember microsomal (hashimotos). anyone know? wow, i need to sleep.

i'm not using goljan, but if i'm not mistaken..

given that microsomal--> hashimoto's ; mitochondrial--> primary biliary sclerosis which is strongly associated with scleroderma (diffuse--> anti-scl-70 and localized (CREST syndrome)--> anti-centromere)

primary biliary sclerosis--> granulomatous destruction of the intrahepatic bile ducts and presents as obstructive jaundice (increased direct bilirubin), hyperlipidemia(leads to xanthomas), pruritus (presumably from bilirubin deposition).

The boards are not going to ask you about Anti SCL(eroderma)70--that's way too easy. Do you know the actual DNA target for these? Likewise for Anti SSa (ro)/SSb(la)?

my buddy and I deem all of these random ass facts as "man vs boy"; if you happen to remember these minute details (as simple as it is), then that'll separate you from the rest of the pack. Granted they wouldn't ask you blatantly what is the antibody, but may be part of lab data that would help to narrow down the ddx

however....anti-scl-70--> i believe is an anti-topoisomerase enzyme (not sure 1 vs 2); the anti from sjogrens' (SSa and SSb)--> not totally sure about the target. However, they may be single stranded binding proteins used in stabilizing unwound DNA during replication (SSB); they may be subtypes of SSB.

Yea, those are right; however, I don't think this is "minutia". It's as much of a minutia as the antimito and antimicrosomal you all mentioned. Anyways, Anti-SSA/B is against antiribonucleoproteins to be more specific, and anti Scl-70 is against Anti DNA Topoisomerase, as you stated.

Also, I know PBC can cause increased incidence of autoimmune disorders, but specifically, it's assoc c scleroderma?

yah, okay...it's really not minutia; these are all important details.....and if we remember them, it'll be the diff between a good score and a great score...

PBS has associations with Sjogren's, hashimotos, RA (i think)....but the strongest association is with Scleroderma. Probably the diffuse form.

I didn't know about that RNP thing; i know that Anti-rnp's are associated with mixed CT disorder (and SLE also, but not as specific)....?

good call on the IgM (is that the isotype of the anti-mito Ab)?

Yea the anti SS antibodies are against ribonucleoproteins.
Hmm...I don't know whether IgM is the anti-mito--will check up on it, don't have my robbins with me.
