I am applying to Pathology, my Step 1 is 232 (94), Step 2 209 (84), but my Step 3 is 185 (75). Does the USMLE transcript include Step 3? if so, do PD's filter by Step 3 too or just by Step 1 and 2?

Your USMLE transcript will include all scores on all steps from all attempts, as of the date of the transcript. If you applied before your Step 3 score returned, then Step 3 isn;t on it (unless you chose the "auto transmit" choice, where a new transcript would be sent as soon as your Step 3 was available).

Whether PD's filter on Step 3 is individual choice.

Seems unlikely as most US seniors haven't taken Step 3 yet.

I think the question is on the definition of "filter". I doubt many PD's create an automatic ERAS filter stating "show me all applicants with a Step 3 score of less than 200". However, when reviewing your application, I think most PD's will take your Step 3 score into consideration.
