Strange insulin regimens

I was wondering if anyone else has seen really unique insulin schedules/doses on patients. I have seen a few lately and I'm trying to figure out if these are super rare or if it's relatively common to have these kind of diabetics. In outpatient, not inpatient

Like for instance huge mealtime doses, 50+ units. Or very frequent dosing throughout the day (more than say, 2 basal and 3 mealtimes) Are these sort of things rare?
