student disciplinary applicant information

In the applicant information section of AADSAS, there is a question in regard to violating a school rule, policy or procedure, or an honor code; or having been disqualified, put on probation, suspended, dismissed, expelled, or otherwise been subject to disciplinary action at any college/university in connection to misconduct? If I was reprimanded but the file was destroyed after 1 year, do I still have to answer this question?

For certain things like cheating, you are required to disclose it, even if the incident never appears on any sort of releasable university record

I would say the severity of the incident and how the situation was handled come into play here. What was the reason you got reprimanded? Was it a formal reprimand?
 
You need to be very careful with this and make sure there is no internal record. Just because it's not on your transcript, doesn't mean there is no record of it with the school. For some dental schools, they will send a form for your school to sign off about academic integrity, after acceptance.. and if there's something there (even not noted on your transcript) your acceptance can be revoked. So just make sure!

Like others have said, check with your school and also check with ADEA regarding their policies.

I was caught using a fake id to enter a bar. I was given a city ordinance violation ticket. I was not arrested. Some of the dentals school admission officers said that they do not care about these types of offenses. I was given reprimand in 2013 and the file was destroyed in 2014.

i checked with my school, they said if dental schools asked they would state I don't have a disciplinary history. During their criminal background check, my incident won't come up because it wasn't even a misdemeanor. I only received a city ordinance ticket

I wouldnt even worry if that was your violation. That like a speeding ticket, it irrelevant in this situation. Just leave it off your application and make sure not to bring it up during your interview.

Used a fake ID to enter a bar, huh. Unless the bar was on campus (which I highly doubt), I wonder why the school was even involved. Anyways, if you were told (in writing) from both your school and the court that you would not need to report this when applying for dental school, then it is fine to leave off. If there is any doubt, I would probably still mention it. One alcohol related incident that happened freshman year or so is not going to be a deal breaker. They understand "kids will be kids".

there are a few bars on campus. The only way a dental school would find out about my ticket would be if they used the freedom of information act. a background check would never show this because it is not a misdemeanor.

You need a lawyer. You may very well be in the system, and that is no small thing. Get one NOW.
