How do you all deal with preps that extend below the gingiva? I like using the rubber dam to keep the field dry but its tough to prep with the rubber dam on when the preps extend below the gums. Also, its tough to keep them dry when it comes to restoring them.

Use a radiosurge or laser to remove the overhanging tissue. If the sub-G restoration is on dentin use a GI or RMGI or (god forbid!) an amalgam

I'd like to try this out, but I'm not sure we're allowed to do this at the school...on my patient I had to abandon the dam and pack cord.
 
On anteriors and premolars I pack cord and use rubber dam with the butterfly shaped clamp to help push gingiva away. So far this has worked well. On molars, just cord. Also, depending on school, you maybe able to use lasers.
 
If the prep is not too large use a skirted matrix... instead of using the normal matrix band use the one with lobes (http://practicon.com/product.aspx?id=40427 --- the one in the middle). Place as you normally would, just make sure you have your position correct.
