I have written a first author publication during a fellowship at the CDC and it is being submitted to the Lancet Infectious Disease Journal next month. Id like to get credit for it in my AMCAS primary but it hasnt been published technically. Is there a way to make a note of it in my AMCAS app or do I have to wait and make an update...or just talk about it in secondaries?

If you haven't even submitted it yet, I think that the best way to handle this would be to describe it as a "manuscript in preparation." Certainly mention it in your essays, and update schools as things progress. I suppose that you could describe the publication in the activities section, but make sure that you mention that it's not accepted yet. Do get it on the primary and secondaries, because some schools won't take updates after that (Stamford comes to mind.)

That sounds good, I was also thinking of asking my mentor to be sure to mention it in his rec letter for me. Just so they can hear about it from a more official perspective?

YES! Do this, I should have mentioned it as well. I had a number of papers that were "in preparation" at the time of my application, but none had been accepted yet. I think that the fact that my mentor mentioned that he expected my work to yield multiple publications probably really helped me out.
