How do you guys decide on whether to submit an abstract to ACC or to complete a full manuscript for publication in an alternative or lesser known journal? Would an ACC poster presentation have more merit than a manuscript when it comes to fellowship applications?

Abstract to AHA/ACC/HRS/ESC/TCT etc. does not hurt in application. But I would always push for a full text paper since it gets into pubmed. Abstracts only appears in google search etc. Good luck!

Anything published always looks better

That being said an abstract at one of these big conferences usually gets an e-journal citation of some sort (not searchable in pubmed) so it is still good. It depends on the rest of your application. Do you have good scores? Go to a good academic residency? Good letters? AMG or IMG? Some applicants can match with few publications, some can not match with 20.

I agree anything published in a journal is better. Most ACC abstracts don't make it to publication. That being said it is relatively easy to submit an abstract to ACC and see what happens. Publications take work.

Submit to ACC
After presenting as a poster, you can go ahead and publish a full manuscript.
Abstracts in ACC is good thing.
But always publication of manuscript > abstracts
