Officially about 60% for non-US/Canadian grads. Cite: ECFMG
Well, there is familiar face. Haven't seen McGillGrad for a min. That 60% must be for all Caribb Grad. mightwant to check out stats individual of the BIG 3. Ross, SGU, Saba. their stats might be alot higher.

Running the numbers quickly, if in 2004 93% passed step 1 for the first time, then only 36% of the 7% that did not pass would eventually fall to the wayside. That means if 100 people originally take then exam and 7 fail, then only 2-3 people out of the original 100 people move on with their lives and do not pass step 1.
I would imagine that they leave medicine or try PA or nursing school.
just a word of caution, these statistics can be fooling. Many caribbean grads drop out of program before they even attempt step I. For example my brother started at SABA with ~40 students in his class and by the time he finished his basic sci there were ~15-20 left in the program and among those ~10 or so passed usmle first time around and the rest took extra time and not sure what happened. I would imagine class sizes at ross and sgu is a lot bigger and the chances of being "weeded out" is higher there and the stronger bunch that's left would eventually take USMLE and ~93% of those would pass it so don't automatically assume that's sgu's average pass rate is equivalent to US MD schools. If you attend a US MD school then there's ~90-95% chance that you will eventually pass everything and become a physician which is a lot higher than any caribbean school.
