Everyone has a different approach, and the best one is the one that works for you. But my 2 cents for what it's worth.

Get Boards and Wards and use it all the way from start of 2nd or 3rd year to the end of internship or whenever. I remember people using it in the 2nd year - something I should have done. I didn't, even though I had it all along. I now keep it in my bag along with the Wash Manual and the more I use it the more I realize that I should have made better use of it during 3rd & 4th year. Get it spiral bound. Know that book back to front and you'll be in great shape for step 2 and 3.

I used Crush for step 2 and 3 -they worked well enough. I had 1st Aid, but didn't have time - Crush was much more efficient - Strong Medicine looked much the same as Crush. For questions I used NMS question book. I looked at the Kaplan one, and again, needed too much time

So a plan:
(1) Boards and Wards from start of MS 3.
(2) Step 2 - Crush Step 2 & NMS question book step 2
(3) Step 3 - Crush Step 3 & NMS question book step 3

For a "picture" book I used (and still do) Color Atlas and Text of Clinical Medicine, Forbes & Jackson.

Green book, ISBN: 1405103418, get it spiral bound at somewhere like Kinko's for about $10.
 
I read very fast, but have a short attention span and benefit from rereading and seeing things in tables and different presentations. What worked for me:

1. Read Crush Step 2 chapter on a topic
2. Red First Aid chapter on same topic
3. Did the really easy questions in the back of First Aid chapter
4. Borrowed NMS Medicine book from a friend (the textbook with the questions, not the question book), and read the sections on the topics I didn't get on First Aid. (This book is huge and unreadable, but useful as reference).
5. Did tons and tons of MKSAP and QBank questions
