I have a friend who is a BE Anesthesiologist with Pediatric & Liver Transplant Fellowships under his belt....He has an active Florida & NY MD License.....

Should he be trying for a job in a rural setup or will a big city hospital consider him... ?

Any suggestions or known vacancies would be appreciated.

I'm not sure what a liver transplant fellowship is, but most big hospital groups are not going to be interested in anyone who they think might not be able to pass the boards. It's often a requirement. Still BE after two fellowships means stop reading the CV. Why take a chance and bring him on only to have to fire him in 2 years when he still hasn't achieved board certification?
