Supervisee/Supervisor Contract

Hi all. I am in the process of transitioning from a prac situation to a hired employee situation and need to have work supervision with a new supervisor. I'd like have a contract with my new licensed supervisor that outlines fees for supervision, expectations from each side etc. Is there a form or template that anyone can recommend?

Any thoughts/ideas/things to look out for would also be appreciated if you were in this situation as well. I am billed as a service provider in the hospital so no separate billing etc. So he's on staff, he'll supervise me if I pay him, the hospital is not requiring it of him but of me obviously as I'm not licensed, and the services are billed as being part of inpatient services. State Board is ok with everything. Thank you for your help!

If he is the one providing supervision to you I would assume he would have a template or form for a supervision contract prepared for you.
 
I'll check with him tomorrow (I'm not sure if he does) but I want to be sure what he has is a typical contract by comparing with others. Thanks!
 
Our state psychological association has all laws and guidelines clearly posted on their website. Part of these guidelines outline exactly all the info that needs to be in such a contract. It generally is the supervisor's responsibility to draw up the contract, but if you are curious about what your state requires in the contract, I would start there.

Thank you! I will look it up at the state level. I was taught to always have my own expertise available when signing contracts, and since I have none in this area, I needed to find others that do have it. 🙂
