It's that time of year again where after we think we're done with applying for applications, we become blindsided by supplemental application requirements from individual programs. Figured I'd start a thread to compile places that require such info. So far,
-UCSD: A secondary app and a statement of why San Diego
-GWU: A paragraph of why GWU
-UAB: picture and eye exam

U of Arizona- $20 ($10 for the university program and $10 for the UPH/Kino program) + a paragaph

question - do these programs automatically ask for supplemental info from all applicants or do they screen first?

(btw, thanks. supplementals for residency applications were a surprise to me.)

To my knowledge they do it automatically. No screening. In fact, UCSD doesn't even notify you to complete the secondary-- you have to look on their website
 
loma linda also wants a paragraph saying why you applied there...
 
Where are people getting this information from? Loma Linda's website says "no supplemental material is required" and the website for the Arizona programs mentions nothing about the "$20 ($10 for the university program and $10 for the UPH/Kino program) + a paragaph" that seanb9 mentioned.
 
I found all the information about supplementals on the SF match program listings. I went through all the programs again after I applied to them to see if they wanted anything else. On that note:
-Tulane: "Tulane app" (http://tulane.edu/som/departments/ophthalmology/residency/apply.cfm) , 2 photographs, and CV
-Temple: Will snail mail supplemental
-UC Irvine: Photo and CV

FYI, most of these deadlines seem to be Oct 1. or close to it.

Also from interview confirmations that programs send - some say what extra stuff they want....

I got two emails (one from the UA program and the other from UPHK, but they came from the same person) that mentioned the supplemental fee and paragraph.

Do the programs send out emails in one batch or at random? I def applied to some of the above mentioned programs, but haven't heard anything abt the supplemental stuff. I am just wondering if the above comments are what is mentioned on the program's website or if people are actually directly hearing from the program RE: the supplemental info? (i.e. GWU, UAB, Georgetown, & temple)

One additional one that I just found out about today is florida - jacksonville needs a paragraph on why you applied there. mentioned on both the website and the sf match listing.

About Temple - do they automatically snail mail it out or do you have to 'express interest' or something - as I havent received anything yet?!
Also UAB says eye exam and photo ONLY If invited to interview, otherwise not...

Sean - the last ones you mention do not mention anything, it is from their website and/or SF match.
But UA you are right - will email and ask for the stuff. There is another thread on which progs send confirmations of application receipt - and some mention it there. Please look on that thread too....

according to UF-jacksonville's website they say "In addition, we also require a statement from the applicants invited for interview in response to the following question: "Why are you specifically interested in pursuing ophthalmology at the University of Florida College of Medicine-Jacksonville?"
 
...but under their sf match listing it states all applicants...confusing😕... I will go with the sf match listing

Anyone received the temple app yet? My appls went out 8-30 and I havent even heard back from them. Supposedly they snail mail it out to you. Emailed too and no response!
Comments/thoughts?!
