Hi all- I just wanted to start a thread because I wanted to make sure I've submitted all my supplemental materials:

So far the ones I know are:

1. Bascom Palmer: 600 word autobiographical sketch
2. UCSD: Miniapplication photo and 35 dollars
3. UCI: Photo, CV
4. Loma Linda: Personal Statement for Loma Linda
5. George Washington Univ: Personal Statement
6. Georgetown: Photo, CV, Personal Statement
7. Emory: Photo
8. Temple: Photo, CV, supplemental app
9. Columbia: Photo
10. UT San Antonio: Supplemental eye exam

do you guys know of any others?? Thanks!
 
Re: the supplementals (such as autobiography and photos), do we send them in now? Or do we wait for the programs to invite us for an interview and then send them in?

I think you need to send them in now for your application to be complete.

Does anyone know how long this georgetown personal statement is supposed to be? i do not get how this is different from what we submitted on SF match...

I think you're just supposed to write a one page essay on why you want to go to that particular program.
 
Just contacted some of these programs and checked out websites to verify.

-Cole Eye (Cleveland): no photo required unless you interview there
-University of Missouri Columbia: same as above
-Pittsburgh: specifically states on their website that you shouldn't send them a photo or any other supplemental information

Hope this is helpful. Good luck to all through the application process...my nerves are starting to feel it! :scared:
 
hmm, the pitt website does have mention that you should not send a photo several times, but their profile on CAS asks for it. their website seems a little out of date, perhaps, with the 2006 interview dates listed. well hopefully they won't hate me for having sent the picture! though if they really don't want it, then I wasted a photo, grr.
 
for bascom palmer's sketch, when i look on the website under the it mentions something about a autobiographical sketch (but it says "handwritten" and doesn't mention 600 words)--am i looking in the wrong place?

look at the sfmatch directory and click on the link "profile." also some of us have received emails from BPEI asking for this document and being told that email was fine.

but for the program director to which i think you are referring, where does one get a username/password..?

you should have been provided one after registration with CAS...but nevertheless...

University of Kentucky is requiring an eye exam (photo optional)...deadline to have it in is Oct 15th...

Looks like their CAS entry hasn't been updated since 2005 so I'd go with the website...although I'm sure you won't get axed just for sending a photo. 😉 I emailed to ask them and let them know there was a discrepancy and never heard back.

On another topic...UF Jacksonville's CAS profile asks for an interest paragraph but it says nothing on their website nor did I receive an email request for it. They've already sent out invites so I'm sure I missed the boat there but thought I'd mention it for future applicants to investigate. Did anyone else notice this? The program's only a year old, right? Kind of hard to find material for an interesting interest paragraph. Ugh!
 
I actually called Pit and confirmed that they only want a picture if they offer you an interview, at which point they'll ask you for it
 
Can we submit these materials via email? I haven't received a reply back from the coordinators...
