Ok so do we print out the supplementary form from the webiste by ourself?

the school updated that additional info is needed!!!

wait... did they contact you? why dont you email them and find out what they mean by additional info needed? the same thing happened to me, and it wasnt the secondary that was needed!

Listen, I'm not "assuming" anything.

The Supplemental Materials page of my AADSAS application has "State of University Questionnaire and "2x2 photograph" listed as "Not received" and I received a letter from Creighton telling me they had all my materials except for my secondary application.

So I'm assuming I'm pretty safe in just sending those in.
