Does anyone know of someone who went into gen surg as a second residency (either after completing another residency or just leaving midway)? If so, did they do a prelim year initially?

Not gen surg but we had an internist come back and do Uro. Prelim built in.
The only reason you would be doing a preliminary year for general surgery is if your application wasn't the strongest. Coming from another field doesn't require a Prelim year.
I met a girl on the trail who did a full peds residency and was now applying to surgery. She said that she had fallen into the trap of talking herself out of surgery even though it's what she wanted. ("You're too nice...what about your husband/family....lifestyle....etc etc.") It sounds cheesy, but it's a good lesson in following your heart.
