A mantra I've heard around our school is "The surgery shelf isn't on surgery". It's a lot of people's experience that a great deal of the questions are about pre and post surgical management. Many people have suggested studying for the surgery shelf like it's an Intenral Medicine exam with a bit of surgery stuff added on.
