Surgical neurophysiologist what are my chances for med schools

Hey all I'm planning on applying for this upcoming cycle and could use some advice if I have a realistic chance of getting into MD or DO or am I living in a fantasy.

I have a 3.23 cGPA and 3.25 sGPA
2 prior MCAT attempts, 496 -> 502 (4 months later, both in 2021). Retaking in 3 weeks, last aamc practice exam I got a 511 (126,128,130,127)

I was a EMT during undergrad with around 4k hours. I have been a surgical neurophysiologist since getting my undergrad in neuroscience and biology with about 7k hours.

I have four years of research in undergrad in the neuroscience department with around 2k-3k hours. Publications include a thesis and two poster presentations.

For ECs I have 15+ years of martial arts and volunteering I have few hundred hours teaching martial arts in an after-school highschool program which included special needs kids. 200-300 hours teaching salvation army workers martial arts. Assisted in teaching local law enforcement martial arts also around 300-400 hours.
Really strong letter of rec from a neurosurgeon and the professor I did four years of research with. Recently got another letter from a ortho spine surgeon that's very well regarded at the few medical schools near me and teaches at them. Originally from ohio but just moved to socal this last oct so still not sure what state of residency I would be either CA or limbo. Still doing research on that.

Overall looking for what people think about my application and if I have a realistic chance or should I take more gap years and do a bridge program.
