Does anyone know exactly how swapping works?

Lets say two people find a place they want to swap with each other. Doesnt the PD of both programs have to agree, then the paperwork and maybe having to interview in the program to see if the person is a good fit, etc?

Basically, for those who've successfully done it. How is the process like?

You are right, neither program director is required to take in a rando that you found on the internet to their residency. But if you found a good fit in both directions and could sell the proposition to both PDs, you might make something happen.
