switching residency program

if you had finished your residency in medicine or in your 3rd year of medicine and want to do a different residency, i know you have to go thru ERAs. In terms of recommendations, who are you getting them from, from the attendings you've worked with during your residency? How do you apply for ERAS, do you have to go thru your medical school again? I'm just a bit confused as to how to even start this process.
