I know the format and limitations vary by journal, but does anyone have a generic template for writing up a case report? I have never done one and have a really good case that I do not see in any current literature.

There is no generic template, and you should probably refuse one if someone offers to send you one. One of the easiest ways to irritate an editor at a decent journal is to not write a manuscript that is consistent with their scope, style, and readership. Just pick up a few recent issues of a few journals you would like to target, and read the case reports that they have published. Those can serve as your templates.
