Questions. Iserson says that we should adress the thank you letter to the PD and then send copies of that letter to all who interviewed us with handwritten notes at the bottom of each to the person you are sending them to. Is it just me, or does it seems crazy to send a letter to Dr. Y when it is adressed to Dr. X?
Any feedback on proper protocol when you had multiple interviewers?

some people say emails, others hand written, and still others say typed. Go with what YOU think is best. You can argue for any of those options, really.
 
Wondering if there are any taboos to making your case /begging to be ranked at the program where you just interviewed?

I wouldn't do this. I assume it would show lack of confidence of being accepted without begging and might seem quite pushy.

I don't see a problem with showing just how much you are impressed with the program and stating that you plan to rank the program #1 if you really want to go there.
 
I think its totally fine to express your genuine interest in the program and indicate you will be ranking them very high. I think it is a bit overboard/redundant to send thank you notes via email to each interviewer especially if its based on a template since many of them may end up in your file and all will look the same (this is also a recommendation of something to be mindful of from our Dean). To prioritize, I would send thank you emails/notes to the chairman, PD, and one of the chief residents. This will ensure your concrete impressions about the program are conveyed and reinforced from the interviews while being filed to remind them who you were when they rank you in January.
