When writing a CV for fellowship, where would you put all the things you did in residency? I know the residency itself would go under education, but what about all the other stuff (chief, teaching, committees, etc).

Also, anyone have a good resource for CVs?

well there are lots of different ways of doing a CV. You should typically put the Chief Position under Postgraduate Training. Though some people put it under faculty positions or honors.
Teaching should have its own section and you should list all the lecturers, courses, seminars, and supervision you provided. Committees should go under service, and can be further broken up into departmental level (i.e. psychiatry), school level (i.e. school of medicine) and institutional level (i.e. university wide appointments). You can also have a section for service to local, national or international (professional) organizations if relevant.

Ask people to send you their CVs and you can get a flavor of what looks well presented and shape your CV that way. That's what I did.

I'd also suggest having a faculty member (or members) review your CV and get their input. As mentioned above, there are many different ways to structure a CV, and you may even have slightly different formats or points of focus for individual programs or whatever it is you're creating the CV for. Some fellowships/awards that I've applied for have gone so far as to request specific headings within which to include the information. There are some general principles that should be followed, but it's not as if there's a written-in-stone set of "rules" that your CV must follow. Organize in your CV in such a way that highlights your strengths and/or the things you believe the program will find important and move from there.
