What should I do if my employer won't pay me for the last few weeks I was there?

This was a NY-based private practice clinic I was at on a part-time basis. I was a practitioner there, and I was supposed to be paid a percentage of my clinic production. I quit my position there without leaving any notice because they weren't paying me anything and kept lying about when they will pay me.

What should I do?

Consult a lawyer, or if the amount in question isn't large, just sue them in small-claims court. You will, of course, have to substantiate what you believe that you are owed.

Department of Labor has a group that assists employees who are not paid, you may want to check out their web site too, dol.gov would be a starting point.

Just as a first step, have you documented what you believe you should have been paid and why? If you're paid hourly, suggest you make a list of days & times worked, or if comp was based on % of practice revenue or something more complicated, do you have a copy of the contract? Have you contacted then by phone or in writing regarding this, if so it would be good to document who and when. Start documenting this in writing; you may want to send a letter with exactly what your request is (be paid for days xyz per the enclosed contract, for example) and send this letter to former employer via certified US mail, return receipt requested. This will prove that you've made good faith efforts to resolve this issue, and will get the matter moving faster.

Most likely once the practice realizes this is not going to go away, they'll want to settle it cheaply; not sure of NY law but in many states you can initiate your own claim in small claims court and do not need an attorney. Depends on the amount in question as well and how important this is to you as it'll require some legwork to get this done.
