Third year chief resident at a community hospital

Hello,

I was wondering if anyone could help shed some light on benefits (if at all) of being a third year chief resident for a community IM program.
I know there really isn’t a role for a third year chief, but nobody has historically stayed on a fourth year chief so we do have 2-3 chiefs out of our class of 10-15.
Essentially it just means more service months and act as resident delegates.

Just wondering if it’s worth it?

Some context, I am a Canadian FMG and eventually had hoped to do heme/onc. Due to the change in regulations on pursuing fellowship as a Canadian, my plan now is to do a 1 year fellowship after my 3 years of IM training, then a three waiver Hospitalist job and THEN apply for heme/onc. I know my chances of fellowship (still requiring a j1 visa) 3 years out of training will be even less than they already are, but I want to try everything I can to help my situation.

All that being said, I don’t know if a third year chief resident position would add more stress and distraction than its worth on a CV.

Would really appreciate some feedback.

In my opinion, it's always worth it as that is a distinction on your CV that you were a chief resident. Also, if i am not mistaken, there is an option on the fellowship application which you can click on if you are a chief resident (Double-check with a colleague to see if that option still exists). Also, being a FMG and a Canadian applying to a semi-competitive fellowship of Heme/Onc, being a Chief resident can only help your cause in the long run. That's my opinion.

3 years of residency + 1 year of fellowship + 3 years of hospitalist + 3 years of fellowship - you must really want heme/onc.

One thing I wanted to highlight to you: you will be 3 years out of training after your J1 waiver hospitalist gig and will still need a visa for fellowship at that point because you won't have a green card by then. My understanding is very few programs give out H1bs so unless you are planning to go back on a J1 (which is probably not possible and also crazy), you might have to wait for your green card which means you are going to be doing more than 3 years of hospitalist work. I wish you the best of luck. I have a feeling at some point during this process you're going to say f*** it.

With regards to your question about chief, I think you should do it.

CCM2017, I'm starting to think I will probably end up being "satisfied" with being a hospitalist at some point along the way. But I wanted to try and do everything possible along the way, in case I want to keep going! You're right, I would again be under a J1 visa... sigh.... let's see!

But I guess if I'll take the position if I am offered it. This is all just speculation at this point but I really appreciate the advice.

Good luck! It may not be possible to go back onto a J1 for training after a Conrad 30 waiver. I would check with ECFMG. Planning ahead won't hurt before setting off on this complicated journey.
