We had a microbiology test this morning covering GI and GU infections (don't even ask how it went). Anyway over the weekend one of the guys in the class wrote up this "PBL Case Study" and sent it out over the listserve. Maybe it's just because I'm knee deep in this stuff right now, but this is one of the funniest things I've ever read. Great medical humor.
Ms. Sally T. Infection a 33 yr old part-time woodchuck breeder / day care worker / and ?adult entertainment consultant? from Greenland cut her foot running barefoot to the mail box to catch the postman in order to get her monthly dues to the Tommy Lee fan club submitted on time. The postman asks her if she wants her additional ?services? today but she declined because she was still coming down from shooting up the heroin. And even if that weren?t the case she still feels ?run down? from visiting the 4th annual Working Girl World Oyster Eating Championships in Thailand. When asked about her relationships she stated that the postman is simply one of her casual friends and they are not serious, the just like to go skinny dipping in the stream by her house.
In addition to the cut, she said she has been suffering from a painful burning on urination, an ?itchy bottom?, slight cough, fever, nausea, vomiting, lethargy, alternating watery then bloody diarrhea, severe headaches, a bad rash, stiff neck and urinary hesitancy that usually start around the same time as the little sores appear on her ?hoo-hoo?. STI states that she is an earth child and only eats raw vegetables from her garden (except for the fried rice at ?The Hibachi Hospital? a Chinese restaurant started by this medical student who needed money for school). She even brings you in some broccoli grown in her garden. When complimented about the veggies, she states the secret is her special fertilizing trick of ?night soil? and the abundance of water that she gets by secretly diverting the treatment plant runoff as a home water supply.
Ms. STI states that she would like some drugs to get her on her feet since she is returning to Mexico to be with the only guy she is thinking of getting serious with since he is a ?keeper? because he wants to be a surgeon after medical school. She is hoping his really bad copious watery diarrhea that he had when she left was better. STI further states that the only downside to this guy is the fact that he is always dragging her to social picnics and making her eat things like turkey tartar, potato salad that ?has had time for the flavor to go thru it?, and rare hamburger steaks with reheated-gravy. STI states that the only thing good about the picnics is the fresh cookie dough homemade ice-cream made with really fresh cream right from the cow. She used to enjoy the square dancing but hasn?t been able to participate because of increasing lack of coordination and a foot drop.
STI states that she is already on antibiotics for her stomach ulcers, immuno-suppressants because of her organ transplant and clotting therapy for her hemophilia. What should you do at the present time?
A)_Tell her it is a self limiting virus and she will be better in a few days
B) Refer her to Genetics given your heightened awareness of her slight dismorphic facial characteristics and multiple consanguity

C) Give her a B-12 shot and have her come back next week in the hope that she brings you more fresh veggies
D) Tell her to avoid the homemade ice cream, as she is possibly lactose intolerant and that is causing the problems
E) Order serology, CBC, cultures and a pizza, because this going to take a while, oh and have Dr. Rank paged to bring the
camera ?just in case?
