Really it's unfortunate the TY year is phased out. It served it's purpose for the right candidates (avoiding arguments about lazy...and if I may add really intelligent ppl). However, since it's integrated with the new Virginia Tech med school, they've had to switch the positions to ED/peds residency spots.
Anyway, my impression is that the intern year is not too bad. One would be expected to be more active in patient care than the TY's would be (mainly because of all the electives the TY's got), but not to a crazy extent.
If you're looking for a 'not so intense' intern year, you may be better off searching 'closer to home' depending on where you are in the country.
However, I did really like the prospect of both programs...great attendings and admins at both. The main thing is that Roanoke is fairly geographically isolated. It is a city, but there's not a bunch going on from an 'outside of work' standpoint. If you're ok with that, then I'd say take the interview. If not, then I'd say let someone else have the interview spot.
The Riverside TY year in Virginia seems the better bet if one is looking for a traditionally cush year as of now.
