Thoughts on Quillivant XR?

Quillivant XR will be the first daily liquid methylphenidate for ADHD. It comes in a 5mg/ml suspension

Right now for short-acting, there is focalin, Methylin, Ritalin and they are given usually BID-TID and last 3-5 hours. For intermediate acting, there is Metadate ER, Ritalin SR, and Adderall which are QD-BID and last up to 8 hours. For long acting, there is Adderall XR, Concerta, Daytrana, Focalin XR, Metadate CD, Ritalin LA, Vyvance, and now Quillivant XR. These last up to 14 hours.

I feel like with a liquid, the dosing errors on Quillivant will be extensive and the cost is a little ridiculous...what makes this really that different?
 
We can give it to our ADHD 2 year olds now and quiet them right up.
 
Because it allows custom dosing, I guess. I see more problems with this than solutions, however.

I only want the newest meds for my children since they must be the best. Liquid? That's great because I can't stand making my children even slightly mildly uncomfortable since they don't like swallowing pills? Oh, the pretty drug rep left you a discount card? Only 10 dollars a month? Perfect. I'll take it for my 4 children and forget to give the discount card to the pharmacist until I've come to pick up in the drive-through. Rinse and repeat.
 
I don't see the point. If you must give amphetamines to a kid too young to swallow pills, and it has to be extended release, why not open up an Adderall XR cap? Who's going to pay for it, and why should they?
 
Amazing how the world has survived this long without Quillivant XR. I don't see it going over big, unless it gets added to a lot of state public aid forumularies (and I doubt it will anytime soon, at least in my state, most XR drugs require a PA....including for generic Adderal XR) Still, I'm sure the company will make a hefty profit off of it, its not like they spent billions in R&D coming up with some novel drug.
 
I sometimes see ADHD meds given to the elderly in nursing homes. Not sure the purpose, but many of these folks do have dysphagia, so if the rx is legit (my big question is what are they being used for in the elderly -- I'm still not sure, but maybe something like cognitive deficits) and they have swallowing issues, that would be one other potential use...

People with early- and mid-stage Alzheimer's are sometimes given ADHD meds, because it makes them more alert.
 
ADHD meds are used a lot in chemo patients to help with their 'cancer brain.' A lot of them are on a no solids diet or have a tube. This will probably be big in the heme/onc area.
 
I guess college students can use it to flavor their margaritas now :meanie:

i see lots of dosing errors to come from this, but as someone else previously mentioned, nice to be able to individualize dosing
