I'm reapplying and I looked over my personal statement and most of it still very succinctly describes my interest in medicine and motivations. I added like three sentences about a new experience in the last paragraph and reworded one or two things, but the majority of it is the same. Is that ok or am I expected to write the entire thing? Anyone have experiences reapplying with the same/almost the same personal statement? If so how did it work out?

If you got lots of interviews the first time, and do not plan on reapplying to the same schools, then I'd say it's fine.

But if you did not get lots of interviews the first time, I'd suggest you take a close look at your PS and see if it could be improved to yield a more favorable result.

And if you're applying to some of the same schools, if they like your app at all, they will see you're a re-applicant and are likely to pull out your last year statement and compare the two to see what's changed since they decided not to admit you. "Almost nothing" is not the right answer...

I'm mostly not applying to the same schools. I'm going to add one or two of the same schools though if I do an amcas application (I'm mainly applying DO). Do you have any advice on how to make a stronger new personal statement?
