Title vs. Position

So I'm putting down the names of my LOR writers and some schools ask for the title of the evaluater, some the position, and some ask for both. Does title just mean say, "Ph.D." and position means say, "Associate Professor of Chemistry"?

NYMC only wants title, what did you all put for that?
 
The title is going to be Associate Professor, Assistant Professor, something like that. The position is Professor of Biology, Professor of Chemistry, Dean of Students, Principal Investigator, Chair of Physiology Department, etc.

I inluded the M.D/Ph.D part in the name, i.e. "Dr. John Smith, MD"

Someone can correct me if I'm wrong, but I think custom is that you either use Dr. John Smith or John Smith, M.D. but never both.
 
Yes, I meant to leave out the "Dr.", but I wrote that in a hurry. Sorry, I hope that didn't confuse you!!
