Too late to ask for a LOR after 2 years?

If she doesn't remember you, you aren't likely to get too strong of a LOR...
 
See if the mentor can write one under the letterhead of the PI. I know people that have done this. It's a better/more personal letter, but essentially comes from the PI
 
If it was one summer, two years ago, and you left without leaving a lasting impression, I wouldn't ask for a letter. I'm guessing you've done things since that time and could ask more recent contacts for letters. Some mediocre letter only saying what project you worked on, probably will not help you when it comes to med school admissions; in fact, it would probably hurt.
 
Adcoms can see the research you've done in the rest of your application. Having someone else restate that isn't going to be a particularly compelling letter. You want to find someone who can talk about you and your traits to show another side of you than someone could see by reading your resume. It doesn't sound like she would be able to shed much light on what you are like as a person.
