That probably depends on the major employers in your area and which companies they use to service their employees. Some of the major players in no particular order are Delta, GIGNA, Aetna, MetLife and there is always Medicaid.

I find Delta Premiere and United Healthcare to have good reimbursements. Other ones generally are pretty low compared to FFS
