I noticed many programs state they accept 3 letters of recommendation. Is our application frowned upon if we send 4 (which is the max we can send via ERAS) ?

Most will be fine with an additional but be sure to read carefully there are a couple programs that say no more than 3 and it may look like you don't know how to read/follow directions if you send too many.

Three good ones are better than 3 with a mediocre one.
Four good ones are better than 3 good ones, but not by much. It does increase the chance that the person reading the letter knows the letter writer.
