IIRC, the admissions officer said they would be on one application and you'd have to designate one or both sites, but giving them a call to check wouldn't hurt.

Not sure about next cycle, but we were given a campus preference sheet when interviewing. You either checked Harlem, Middletown, or Either (list first choice).
