Hand wrote the app and typed the essay.
Thank you. Also, did your app come a little beat up? It looks like my envelope went through a war zone.
Mine had to get all the way to Austin, TX; the envelope was so worn at the edges, everything was about ready to fall out.
For the fields that are in gray, should we fill those out or are those for office use only? Because on the top right of the application "Office use Only" is highlighted in gray, so i'm assuming everything else that is gray throughout the application should not be filled out by us??? ie. Date of birth and Optional questions
I forget about the grey boxes...what are some of the other questions?

In my opinion, it looks like if it doesn't say for office use only and it's still grey then it's because it's optional and will only be used for statistical and informational purposes. Unless you were born in 1930 and don't want that to be known I would fill it out.
Can we just fill out a typed over version of the form available on the web or do I have to wait to receive the secondary in the mail? Any difference between the two forms?

You can fill out the secondary they have online and just send it out when you receive the secondary in the mail. That way you will have a head start. There is no difference between the two forms, its all the same questions. I sent in the online app they had on their website.
