If measure blood levels of TXA is the same topical or IV then wouldn't it theoretically be the same risk? In that case why give topical over IV just give it.

The levels weren’t the same from what I recall but they were closer than I would’ve thought. I think you’re right though that risk (and benefit...) appears the same regardless of admin route. Also just FYI, as the guidelines state basically IV=topical the alternative argument of giving it all topically could be made.
 
not to beat a dead horse but we probably do 20-30 total joints at my place a week all with TXA (pretty much regardless of comorbidities including stroke, CAD etc) . Have not yet seen a transfusion for a primary knee or hip. Also rare TQ use for knees.
