Hi Everyone,

Does anyone have copies of the Transcript Matching Form and the Evaluator Matching Form from last year? If so, would you mind sending me a PM? I know these are supposed to be accesible through AADSAS, but for some reason you can't see them at this time.

These forms will be specific to your application and have a correlating bar code on them. This allows for your paper work to be identified and matched to your application. So, I do not think that it will do you any good to have these forms from last year.

Thanks, USCbiograd. I'm I was not aware of the bar code. I thought it would simply be a standardized form. I appreciate the info.
