Transcript matching form

I read we have to send a matching form w/ our transcripts to update this past semester's grades. But I cant seem to find it. I've tried googling it and get a form for 2005, can someone please help? thanks in advance.

Not sure if this answers your question:

I think the transcript matching form is in your aadsas account (in the colleges attended section)

Log into you aadsas account
Click on the (transcrip matching form) button from the checklist
It should say colleges attended at the top of the screen.
Then there is a red lettered phrase with an astrik that says required field
Under that is the light blue box that says Transcript matching form.
Under that is a light grey box with a blue arrow surrounded by white and bordered on the left by red.
Click on that arrow
Your personalized form should come up.
