I have two universities i am not close to that i will be requesting transcripts from. I will just be saving the form as a .pdf and then emailing it to whoever i talk to when i request my transcripts. Worst case scenario i will fax it to them.

Some universities have an attachment option to upload an additional form while ordering yuor transcript. Otherwise, I called the school to explain and faxed it.

Yes I think I'll do this. Emailing as .pdf seems to be the fastest method.
