Can we fax the transcript matching form to the college or do we have to mail it to them? I currently live far away from my past college...Fill it out, and hand them to the transcript office at your school.

I thought the blanks became automatically filled by typing up the application?
Are you guys filling out "Former Name Used While Enrolled in the Institution" even though it is already filled out under "legal name". Or is this only for people who have different names in school ?

Fax. Email. Just correspond with them so they know what to do.
Have the transcripts mailed to you. Then put that envelope + matching form in a BIGGER envelope and send them together yourself.

I was thinking of doing this too as I already requested my transcript a few days ago, but than I read this:
The transcript must meet the following requirements to be considered official by ADEA AADSAS.
· Registrars seal and/or legible signature included on transcript
· Be received directly from the Registrars office
· Transcript NOT marked issued to the student or student copy

Do NOT do this.
You will risk delaying your application. The transcript must be sent directly from the registrar.
