Transcript Request Form Question

I've been trying to find this information on previous threads, but I haven't had any luck. On the application, it mentions to print the transcript request form and then sign it. Should I send this request form to the college(s) that I have attended and then they will then submit my transcripts to the ACMAS?

Not totally an expert on this as I'm figuring out how to do this right now too... but I went on my universities "order transcript" system from my student page and from the dropdown menu I could select AMCAS and it popped up with all the addresses correct and had a box that read

"If you select AMCAS from the dropdown you will not need to send the transcript request form from your application"

All I needed for my universities system was my AAMC ID and the Transcript ID that came with my application.
 
Follow your university’s procedure. Call their transcript office if you can’t find the info online. At my university, for example, one needs to order a transcript through the regular channels and then send in the request form, they won’t send it otherwise if you just give them a form.

It depends on the University. I have one school where their electronic system asks for the AAMC and Transcript ID so they don’t require the form, but then my other school I have two options: order online then email the registrar with my order number and the ID’s so they can attach them, or print out the order transcript form and mail to them along with the transcript request letter.
 
I was wondering about what to do if I attended two different schools within the same university system, so that the transcripts are identical for each school - i.e. the transcript for school 1 reflects the coursework I did in both school 1 and school 2, and vice versa. It seems silly to submit two transcript request forms and have each school submit the exact same transcripts to AMCAS. Anyone else have experience with this specific situation? I just want to make sure I am lining up what I list in my AMCAS application with the transcripts I'll be submitting to AMCAS and to make it as easy as possible for AMCAS to verify my transcripts, but the application doesn't make it easy to indicate all of that.
 
So you attended two campuses of the same school and both are on 1 transcript?
You can list school A and then School B with a transcript exception on the second school

Yes, exactly. Thanks for responding to my question. That makes more sense, though it seems like jumping through the hoops of a transcript exception is more annoying then just requesting a second transcript. If I'm okay with eating the cost of submitting two identical transcripts and having each transcript linked with the proper transcript ID, that should do the trick as well, right? (I apologize in advance for the neurotic questions)
