I know it's probably a little late and most people have already applied...
I have had huge trouble getting one school to send transcripts, and when they said they did, they still haven't linked with my app. So I created the attached form.
It's literally a copy of the AMCAS form. Hence the horrible color scheme, but it works fine if you print in B&W. If anyone wants to use it I think it should be useful for future app cycles, I tried to keep it adaptable.
Use it if you want, feel free to ignore.
