Transcript Request Forms...Send by itself? Add instruction page?

When sending transcript request form did you guys just send the form by itself? Some people said they attached an instruction page but is doing so really beneficial?
 
I attached the page, but I have heard of people that haven't and didn't have trouble. If I were you, I'd follow the instructions as much as possible. Applying to med school is serious business.

i might not have made it clear, but from your response it seems as if there's an instruction page we can attach in addition to the Transcript Request Form itself?
 
Oh i see, you're saying that though not recommended, some people just have their undergrad send transcript to aacomas and amcas without TRFs?

I don't precisely remember what it was called, but it was a cover letter that is meant to be attached alongside the transcript.
 
Every school has a different transcript request process. I was getting transcripts from 3 institutions. All registrar's offices are familiar with the concept of attachments. If your registrar's transcript request web site or form doesn't indicate what to do, just call and ask.
 
I didn't attach the form and both of my transcripts were received about 2 weeks after I ordered them. When I sent it though, I sent it to AACOMAS - TRANSCRIPTS, My AACOMAS ID#. I am no longer at either of the 2 universities so that's why I did it that way, I would have handed in the form if I was on campus though.
 
AACOMAS specifically says that if you can't attach their form, you can put your AACOMAS ID in the address field. I believe AMCAS requires the cover page.
 
I'd call and ask. Most schools do some form of electronic requesting, which might be easier than mailing a form. I was able to email the attachment to one of mine, and they attached it no problem.
